We are currently seeking applications for the role of Support Coordinator in the NDIS department. Applications are open till 9am, 8th October 2026. Position Objective The Support Coordinator is responsible for assisting NDIS participants to understand, implement and make the most of their NDIS plans. This includes connecting participants with appropriate providers and mainstream services, negotiating service agreements, managing plan budgets, and building the participant's capacity to self-direct and manage their supports independently over time.The role requires a participant-centred approach, working collaboratively with each individual to understand their goals, preferences and circumstances, and to coordinate a network of formal and informal supports that maximises the value and effectiveness of their NDIS funding. The Support Coordinator ensures that all coordination activities are carried out in accordance with the NDIS Practice Standards including the Core Module requirements for rights and responsibilities, governance, provision of supports, and the support environment and that participants receive transparent, factual advice that promotes genuine choice and control. Main Duties and Responsibilities Support Coordinators at Allambi Care play a multifaceted role in assisting participants with their support plans. Their responsibilities include: Assessing Options & Linking to Services - Evaluate mainstream, community, informal, and provider options available to each participant, ensuring options are tailored to the participant's individual needs, preferences and circumstances.
- Connect participants with mainstream or community services such as housing, education,
- transport, and health to complement their NDIS-funded supports.
- Provide information about support options using the language, mode of communication and terms the participant is most likely to understand.
- Ensure each participant is aware of the full range of options available to them, including
- both registered and unregistered providers where appropriate.
Choosing & Negotiating Services - Assist participants in selecting their preferred support options or providers, respecting the participant's right to choice and control.
- Negotiate the services to be provided and their costs, ensuring value for money and alignment with the participant's goals.
- Develop service agreements and create service bookings with chosen providers
- Negotiate services and prices for any quotable supports.
- Assist participants in understanding their responsibilities under service agreements. Help participants change or terminate service agreements as needed.
Budget Management - Decide on the budget allocation for each type of support in consultation with the participant.
- Inform the relevant plan manager about the breakdown of funds.
- Collaborate with plan managers to establish appropriate claim categories and allocate funds correctly.
- Monitor plan budgets and support effectiveness throughout the plan period, ensuring supports are used efficiently and for the purposes intended by the participant.
- Support participants to understand budget flexibility and how to maximise the value of their funded supports.
Capacity Building & Promoting Independence - Enhance the participant's ability to coordinate, self-direct, and manage their supports over time.
- Assist participants in resolving any issues that arise with their supports or providers.
- Progressively build the participant's skills and confidence to manage their plan independently, with the primary objective of reducing the participant's need for support coordination over time.
- Support participants to understand how to participate in NDIA planning processes, including establishing service agreements and managing budget flexibility.
- Empower participants to take more control over their support coordination, fostering autonomy and self-advocacy.
Plan Reviews, Assessments & Transitions - Negotiate services, costs, and develop service agreements and service bookings with chosen providers.
- Organise necessary assessments to determine the type and extent of funding required (e.g., complex home modifications, functional capacity assessments).
- Assist participants in changing or terminating service agreements as needed.
- Assist participants in preparing for their plan reviews by evaluating goal achievements and value for money.
- Identify and address any problems encountered in plan implementation.
- Help participants consider new goals and emerging support needs.
- Support participants in exploring housing options and planning life transitions.
- Support smooth transitions to or from the provider, ensuring continuity of supports.
Risk Identification & Incident Response - Identify, analyse, respond to, and manage risks to participant wellbeing, including risks related to support breakdown, safeguarding concerns, or changes in circumstance.
- Report and respond to incidents in accordance with Allambi Care's incident management system and NDIS Commission reportable incident requirements.
- Support participants to understand how to raise concerns, provide feedback, or make a complaint at any time.
- Escalate high-risk or complex situations to the Support Coordination Manager or Specialist Support Coordinator where appropriate.
Documentation, Compliance & Quality - Maintain accurate, timely and comprehensive records of all coordination activities, participant interactions, service agreements, budget tracking, and communications.
- Ensure participant information is stored securely and managed in accordance with privacy legislation and Allambi Care's information management policies.
- Participate in Allambi Care's continuous improvement, incident management, complaints management and quality assurance systems.
- Ensure all work is conducted in compliance with the NDIS Practice Standards (Core Module), the NDIS Code of Conduct, and all relevant legislation.
Conflict of Interest Management - Provide transparent, factual advice about support options that promotes genuine participant choice and control.
- Where Allambi Care has an interest in any support option available to the participant, ensure the participant is made aware of this interest and understands that their choice of provider will not impact on the coordination service.
- Ensure conflict of interest policies are explained to each participant in the language and terms they are most likely to understand.
- Document all referrals to and from other providers for each participant.
- Register any identified conflicts on Allambi Care's Conflict of Interest Register as required
Essential Requirements - Strong knowledge and understanding of the National Disability Insurance Scheme, in particular support coordination processes and relevant NDIS regulatory frameworks.
- At least 2 years’ experience working with people with a disability including children and young people, or in a similar environment to Allambi Care.
- Relevant experience in service quoting, understanding of the NDIS Price Guide, and provision of support work for people with a disability.
- Demonstrated ability to identify, analyse, respond to, and manage risk, incidents and complaints positively and proactively.
- Demonstrated ability to implement and participate in performance management systems designed to create a positive organisational environment that values the contributions of each individual employee.
- Promotes the importance of collaborative work to achieve greater motivation and outcomes for participants.
- Proven abilities in negotiation, problem solving, and conflict resolution.
- Demonstrated evidence of excellent time management and organisational skills, and ability to prioritise tasks.
- High level of computer literacy in using a range of computer systems including Microsoft Office applications.
- Demonstrated high-level written, verbal and interpersonal skills.
- Ability to obtain the information relevant to the individual's circumstances and document discussions for the planning process
Desirable - Relevant tertiary qualification in a discipline such as Social Work, Community Services, Disability Studies, Psychology, or an equivalent field.
- Experience delivering support coordination within the NDIS.
- Knowledge of trauma-informed practice, restorative frameworks, and strengths-based approaches.
