NDIS - Community Participation Coordinator (Fixed Term)

We are currently seeking expressions of interest for the role of Commmunity Participation Coordinator in the NDIS department for a Fixed Term of 12 months.

Position Objective

 

The Community Participation Coordinator is responsible for the planning and delivery of the NDIS community services, including in-home supports, community participation and mentoring to ensure all relevant activities occur in a systematic, coordinated and planned manner.

 

They will ensure the development of the staff in line with NDIS guidelines and best practice to ensure that community services are tailored to individuals, and delivered in a manner that creates an environment that is empowering for people with disability and their service needs.

 

Main Duties and Responsibilities

 

SERVICE DELIVERY

  • Coordinate the implementation of Community Service supports in line with the needs and goals of participants and overarching NDIS operating guidelines with a focus on maximising the utilisation of NDIS funding for participants, respecting participant rights to choice and control, and achieving constant quality outcomes:
    • Codesign and coordinate the implementation of supports that promote, uphold, and respect the legal and human rights of people with disability.
    • Oversee the provision of supports to ensure they promote, uphold and respect individual rights to freedom of expression, self-determination, and decision-making.
    • Show ability to identify, analyse, report, and respond to risks to participants, workers, and Allambi Care.
  • Manage the triaging of requests for Community Service supports by identifying potential caseload vacancies, considering the participant to staff match and engaging with participants and/or their designated decision-maker to discuss their needs.
  • Develop ongoing working relationships with Home & Living participants, their families, nominees, Support Coordinators, and any other key stakeholders to ensure open and regular communication in line with NDIS Supported Decision Making Policy to ensure all decisions and/or issues can be resolved in a timely manner.
  • Collaborate with external agencies and disability stakeholders, including but not limited to, NDIA, NDIS Quality and Safeguard Commission, NSW Trustee & Guardian, and Support Coordinators, to build ongoing working relationships that can ensure quality outcomes for participants.
  • Provide direct oversight of the quality and standards of claiming and supporting evidence captured through the NDIS App for supports delivered by the Community Services team by:
    • Ensuring the completion of necessary service agreements and programs of support in line with required standards
    • Ensuring all service agreements for engaged participants are obtained and stored in AIMS in line with privacy and confidential policy.
    • Coordinate the implementation of rostering in line with participant needs and available funds.
  • Contribute to strategic planning and decision making to ensure ongoing sustainability and success for all Community Service supports.

 

GOVERNANCE AND COMPLIANCE

  • Maintain knowledge and understanding of current NDIS legislation and operating guidelines, with a particular focus on key policies, practices, and documents related to Community Participation services:
    • Oversee and direct understanding about the limitations of the NDIS and where mainstream or other government supports or services may be more appropriate.
    • Oversee and direct clear, proactive communication with internal and external stakeholders and participants about the changes in NDIS rules and guidelines and how they may affect the provision of supports.
  • In conjunction with the Planning & Compliance Manager and EOO, develop, implement, and oversee objectives, policies and procedures related to the delivery of AC Community Service supports to ensure Allambi Care continues to meet registration standards and requirements, and:
    • Ensures that people with disability are free from violence, abuse, neglect, exploitation, or discrimination.
    • Ensures Community Service supports that focuses on participant human rights, choice and control and supported decision-making.
    • Ensures each participant exercises meaningful choice and control over their supports and maximises the value for money they receive from their supports.
    • Identifies and minimises conflict of interest and ensures each participant receives transparent, factual advice about their support options which promotes choice and control.
    • Explore and navigate holistic supports and services that are in line with NDIS reasonable and necessary criteria and develop and maintain both formal, and informal supports.
    • Ensure these align with NDIS guidelines, Allambi Care’s Needs Based Restorative Framework, and best practice.
  • Keep up to date with discussions in the sector around proposed policy changes relating to Community Service support and proactively contribute to strategic planning for Allambi Care’s Community Service program.
  • Follow any changes to AC policies and procedures are communicated and trained to their direct reports.

 

SUPERVISION & TRAINING

  • Provide leadership, direction, supervision, and ongoing feedback to staff within the Community Services team and promote work practices that are consistently aligned with AC Values and the principles of the NDIS, such as upholding human rights, celebrating diversity and respecting the voice of participants and/or their decision makers.
  • Engage in the development and delivery of ongoing training related to NDIS Community Service supports to ensure the organisation and its staff remain informed and supported to continue to provide services that align with disability best practice:
    • Clearly communicate objectives, policies and procedures and train these to staff in the Community Services team.
    • Ensure that Community Services team are trained in and supported to meet the standards required for reports, submissions and evidence when completing administrative duties in line with NDIS requirements and timeframes.
  • Create a positive organisational environment with clear communication pathways which values the contributions of each individual employee and promotes the importance of collaborative work to achieve greater outcomes for participants and staff motivation. This includes the facilitation of Community Services team meetings, as well as group supervisions.

 

CONTINUOUS IMPROVEMENT

  • In conjunction with the NDIS Senior Management team, participate in Quality Assurance and Continuous Improvement processes by recommending changes and communicating issues that are identified.
  • Alongside NDIS Senior Management, engage in the development and implementation of performance based KPI’s within the NDIS department:
    • Provide regular updates to the Director – NDIS to measure progress against required outcomes that communicate the effectiveness and progress of NDIS services:
    • Oversee and participate in performance management systems that ensure Allambi Care services are delivered by employees who are competent in relation to their role and who have relevant guidance, expertise, and experience.

 

SAFETY AND RISK MANAGEMENT

  • Promote a work environment that is safe, healthy and complies with WH&S standards, and model work practices that always align with WH&S policy and procedures.
  • Oversee the Allambi Care incident management system, ensuring that incidents are acknowledged, respond to, well-managed and learned from.
    • Ensure critical incidents are dealt with according to AC Policies and Procedures and legislation whilst keeping positive outcomes for participants at the forefront of any decision making.
  • Oversee a complaints management system that ensures that complaints and other feedback made by all parties are welcomed, acknowledged, respected and well-managed.
    • Work alongside People, Support & Culture, to respond to any issues, complaints or grievances in line with AC policy and procedures in a suitable and professional manner.

 

OTHER DUTIES

  • Engage in other duties which may be required, requested, or directed by Director - NDIS or the Executive Team and which are within the employees capability and training to perform.

 

Minimum requirements

  • Strong knowledge and understanding of the National Disability Insurance Scheme, in particular supports that are consider community services, processes and relevant NDIS regulatory frameworks.
  • At least 3 years working with people with a disability including children and young people, or in a similar environment to AC.
  • Relevant tertiary qualification or and relevant extensive experience in service quoting, understanding of NDIS price guide, roster development, and provision of support work for people with a disability.
  • Demonstrated ability to identify, analyse, respond to, and manage respond to risk, incidents and complaints positively and proactively.
  • Demonstrated ability to implement and participate in performance management systems designed to

o   Create a positive organisational environment that values the contributions of each individual employee.

o   Promotes the importance of collaborative work to achieve greater motivation and outcomes for participants.

o   Ensures Allambi Care services are delivered by employees who are competent in relation to their role and who have relevant guidance, expertise, and experience.

  • Experience in the supervision and management of employees.
  • Proven abilities in negotiation, problem solving, and conflict resolution.
  • Demonstrated evidence of excellent time management and organisational skills, and ability to prioritise tasks.
  • High level of computer literacy in using a range of computer systems including Microsoft Office Applications
  • Demonstrated high level written, verbal and interpersonal skills and a proven ability to prepare reports for Executive Management.
  • Ability to obtain the information relevant to the individual’s circumstances and documenting discussions for the planning process.
