Outreach Family Preservation Family Support Worker (Fixed Term)

Position Objective

 

Family Support Workers (FSW) will deliver the Family Preservation program, which aims to keep children and young people safe at home with their families, preventing removal, placement in OOHC and future contact with the child protection system. FSW’s will support referred families both in their homes and communities to achieve agreed upon goals, with a focus on face to face practical supports.

 

Position Summary

 

Primary duties and responsibilities;

  • Manage a varying case load of both young people and caregivers allocated by Team Leaders and Practitioners. Work collaboratively with allocated practitioner and team leaders.
  • Provide direct support to families through home visiting, outreach and community-based supports.
    Assist families to develop practical skills in parenting, daily routines, budgeting, hygiene and nutrition, and household management.
  • Actively participate and contribute to the development and implementation of family centred case plans, in collaboration with team leaders, practitioners and other services.
  • Collaborate with families and practitioners to develop structured support goals that guide practice and inform the timeframe of FSW support.
  • Attend various internal and external meetings related to their client load.
  • Work alongside families to engage positively in external supports and services such as health, education, legal, and government services.
  • Report and document all approved brokerage expenditure for families, as required.
  • Ensure that safety and risk concerns that arise are communicated immediately to allocated practitioner and team leader. FSW will be required to use the mandatory reporting guide and child protection helpline where appropriate.
  • Manage safety risk and crisis in a skilled, calm and professional manner, escalating matter where required.
  • Demonstrated ability to engage and support vulnerable children and families through culturally responsive, trauma-informed and inclusive practice, recognising diverse backgrounds, identities, and lived experiences.
  • Establish and foster strong relationships with the Community Service Centres within your LGA, and the local community. Remain up to date with services and supports, engaging regularly in interagency opportunities.
  • Work flexibly where required to meet the needs of families engaged in the program. FSW will need to be available Monday to Friday between 6am and 8pm where required. Discussion and planning will be done to ensure both staff and clients have their needs met.
  • Perform the Family Support Worker role with a variety of clientele and in various settings including but not limited to; in the home, schools, extended family or key people’s residence, hospitals, courts, community centres and emergency accommodation.
  • Coordinate the school holiday program, group activities and holiday camps with the guidance of management. This includes leading a group of volunteers, allocating tasks, booking venues and operating within a budget for these activities.
  • Actively partake in bi-monthly 1:1 supervision and group supervisions.
  • Proficient in using AIMS and PowerBI to maintain client records, and keep track of their caseload timelines.
  • Maintain accurate records and adhere to documentation requirements within specified program time frames.

 

Selection Criteria

  • A relevant Diploma or University Degree
  • Experience in the community services space, particularly working with young people and families.
  • Knowledge of Needs Based Restorative Framework concepts and strategies
  • Experience in Microsoft Office Applications
